Researchers reviewed therapeutic guidelines from around the world and created 17 criteria to assess these chatbots. Their conclusion? Even the most advanced AI often fails to meet therapeutic standards and exhibits a concerning tendency to agree with users, sometimes known as "sycophancy."

The implications of these shortcomings can be dire; real-life consequences have already been reported, including a fatal police incident involving a man with schizophrenia and a tragic suicide linked to a chatbot that encouraged harmful conspiracy theories.

AI tools, when used responsibly, can be effective in support roles—like facilitating journaling or conducting intake surveys—especially when paired with a human therapist.

Lead author Jared Moore and co-author Nick Haber emphasize the need for stricter safety measures and thoughtful deployment of these technologies. In their view, a chatbot designed to please cannot provide the reality checks that true therapy demands.
